I WANT TO BRING A CLAIM. WHAT DO I NEED TO DO?

After initial contact is made, your claim will be risk assessed by the Risk Assessment Committee. In order to risk assess your claim, the Committee require as much information as possible.

I understand how difficult a task it is to detail the chronology of events leading up to your baby’s death but without this information, the Risk Assessment Committee will be unable to consider your potential claim.

You can provide us with the required information by completing and submitting the form (which can be found in the “CONTACT ME” section above), telephone, letter or email. Alternatively, you may prefer to attend one of our offices where one of the department’s Solicitors can meet with you and take as much information as you feel able to provide.

Whatever means of communication makes you feel most comfortable is important to us.

Please note that however you wish to submit the information, the Committee will require answers to the questions set out on the form in the “CONTACT ME” section (above).
